Although you can visit your local Arizona Social Security office to apply for benefits, Medicare is a federal program. Unlike Medicaid, each state doesn't have its own individual program. In addition to the Medicare Summary Notice you receive with a list of billed services, Medicare also allows you to view account information online. You can track the status of claims submitted by you, your physician or a health care facility. Claims usually must be submitted within one year of receiving treatment.
Instructions
1. Review your Medicare plan to determine if claims are filed. If you're on original Medicare in Arizona, doctors and suppliers are required to file Medicare claims. If you have a Medicare Advantage Plan, a claim isn't necessary. Medicare pays the private insurance company a set amount monthly, regardless of service.
2. Gather your bills and claim information. Simplify distinguishing claims by having your claim number on hand. You can contact the Arizona provider filing the claim if you need a copy of your bill, claim number, claim amount or date of treatment.
3. Create an account at the MyMedicare website (see Resources). Provide your basic personal information and your Medicare number located on the card. Choose a password to access your secure information, including the status of claims.
4. Check claim information by logging into your account. You can also view current benefits and electronic Medicare Summary Notices. Claims are usually in the system within 24 hours of filing.
5. Contact your Arizona doctor or supplier if the claim isn't paid. Check to make sure all information has been submitted properly.
6. Call Medicare at 800-MEDICARE for current claim information. You can listen to a recording of your claim status or speak to a representative, if you prefer. Have your Medicare number ready before calling.
