Requesting a copy of your medical records is not difficult.
Under the Health Insurance Portability and Accountability Act (HIPAA), residents of all states, including North Carolina, have a right to get a copy of their medical records. The state of North Carolina law also has requirements regarding how doctors, hospitals and health care providers in the state must keep and make your medical records available. Under state law, medical providers in North Carolina are allowed to charge you a reasonable copying fee.

Procedure to Request Your Medical Records
1. Ask your health care provider about the procedure you must follow to request a copy of your medical record.
2. Use your provider's request form if possible.
3. Make your request in writing if your medical provider requires it.
4. Include your name, address, telephone number, date of birth, date of the medical procedure you are requesting, and a description of the information you want to see, such as X-rays or specific test results.
5. Specify whether you want to view your medical record, would like a copy of it, or both.
6. Allow your health care provider 30 days to make your records available, or 60 days if the records are kept off site.
