Apply For Tenncare

TennCare provides health insurance for low-income residents of Tennessee.


Over 1.2 million residents are covered by Tennessee's state Medicaid program, known as TennCare. This program provides needy individuals with access to medical care they would not otherwise be able to afford. If you are pregnant, a single parent or caregiver, have a disability or have lost your job, you may be eligible for benefits as long as you are a current resident of Tennessee. You can obtain an application from your local Department of Human Services.


Instructions


1. Visit the eligibility section of the Tennessee Government's TennCare website to determine if you quality for assistance. The set of requirements are different for certain individuals and programs. Look under the section for the program you wish to apply for. TennCare programs include Medicaid and TennCare Standard, which is only available to children ages 19 and under.


2. Locate your local Department of Human Services office. You need the address and phone number of your local chapter to go in person to pick up an application. Alternatively, you need this information to mail or fax a printed application form after it has been completed.


3. Obtain an application for the TennCare program. You can pick up an application form in person from your local Department of Human Services. A printable form is also available on the Tennessee Government's website.


4. Complete the entire application of benefits with valid information. The application requests basic personal information including your name, address, phone number and Social Security number. You must add every member of your household to the application form in the designated areas.


5. Send the form to your local Department of Health and Human Services office. You may drop the application off in person, mail or fax the form. After submitting your form, wait for a follow-up call or letter in the mail. This letter or call notifies you of your official eligibility and contains instructions for obtaining your benefits.
